To make your visit with us faster, you can print and complete the Intake Form [PDF, 39KB] ahead of time. Be sure to bring your:

1. health insurance card/s,
2. prescription (from your physician)
3. driver’s license or valid ID and
4. letter of medical necessity (from your physician) so that we can help complete your health insurance forms.
