We will ask you to sign several forms, as you would at any medical care provider. Please bring with you your:

1. health insurance card/s,
2. prescription (from your physician)
3. driver’s license or valid ID and
4. letter of medical necessity (from your physician) so that we can help complete your health insurance forms.
